Sweet Tooth eventually pursued her to a hospital, where he wiped out numerous patients, doctors, and police officers in his search for her. This would eventually grow into an obsession that would drive and motivate him to hunt her down. Sweet Tooth had grown angry and bitter due to him not being able to successfully kill his daughter. The two then climb in Tower Tooth, powering it on. Confused, Marcus wondered why this all happened, but Sweet Tooth persuaded him to forget it. Marcus asked who he was, but Sweet Tooth replied that he was Marcus. The two encounter the nonoperational Tower Tooth, a humongous and terrifying modification of Sweet Tooth's truck. In his alternate ending as Dark Tooth, he meets up with his split personality, seemingly, having now been separated. Calypso tried to get his guards to kill Sweet Tooth, but Sweet Tooth made them kill him instead. The two suddenly changed into each other. Calypso said he would never grant such a wish, but Sweet Tooth pointed out that he had no choice. He then made his wish, that he and Calypso would change places.
SWEET TOOTH DRIVER
Sweet Tooth said he'd been getting tired of his job as a simple driver and noted how they'd been going through the same events year after year. Calypso was their waiting, asking what his wish was this time. In Sweet Tooth ending, he crashes into Calypso's lair. This means that while events in Twisted Metal: Black could have happened, they definitely twisted or altered in some way, due to Sweet Tooth's insanity. *Twisted Metal: Black is confirmed to take place inside Sweet Tooth's depraved mind. Often twisting their words to harm them in some way. He also had Calypso's twisted sense of humor when it came to granting the request of the winner. Sweet Tooth also served as the final challenge to any competitor who made it to the end with his upgraded ice cream truck. He was now the ringmaster instead of the clown, running things from his carnival like lair with his horde of maniacal clown minions. Thus, when Sweet Tooth won the tournament, he used his wish to take Calypso's place as the head of Twisted Metal.
He realized that Calypso was just using him as a pawn to collect souls and become stronger.
SWEET TOOTH PROFESSIONAL
Sweet Tooth proved to be an extremely skilled competitor being a professional murderer, taking out many opponents, but it soon lost its appeal. He had constant visions of death and mayhem and continued to follow the tournament for years until he finally was able to become a competitor. When Sweet Tooth was a child, the Twisted Metal tournament took place in his town and had an extreme impact on him. Sweet Tooth serves as the main antagonist in the game. However, the pain caused by the fire would drive him further into insanity. He then presumably escaped his containment. Needles did not die, rather, he awoke with his head constantly on fire, causing him strong pain and seemingly becoming immortal.
It is here that as he was getting ready to be executed, a man who appeared to be a priest came forward and shouted various hexes and curses at him. This would culminate in him getting arrested and sent to the electric chair.
SWEET TOOTH SERIAL
Eventually becoming one of the deadliest serial killers of all. He would go on to murder countless individuals. He also let his son run off, although badly scarred, in order to have a successor. Needles murdered his wife, baby daughter, and attempted to kill his daughter Sophia, however, she was able to escape his grasp by stabbing him in the eye with scissors. He carved his iconic mask, meant to look like the mascot of the company he worked for, and became the man known as Needles Kane. This other personality wore away at him, until he finally snapped. Inside, Marcus was feeling a burning rage and hostility that he repressed into a dark, split personality. Marcus was seemingly a normal happy man living a normal suburban life.
